Booking your Appointment with the Nurse
- Travel Risk Assessment:
If you're planning a trip abroad please complete a Travel Risk Assessment form (available from reception) with details of your proposed journey. Alternatively if you prefer, drop into surgery to complete a form. (Kindly note that this service is for registered patients only) - Travel Appointment:
Once you have completed your online assessment form it will be reviewed by our Practice Nurse(s) who will contact you to advise your vaccination requirements and to book an appointment. If you have not been contacted within two weeks of completing the travel risk assessment form please telephone the surgery. - Please Allow Plenty of Time:
European Destinations: Appointment will then be given within 4 week time frame from the point of contact
Outside Europe/Multiple Destinations: Appointment to be given within a 6 week Time Frame (To allow completion of complex vaccination schedules e.g. Rabies to be ordered and completed)
No Travel forms will be taken or appointments given before this time frame
Patients can be offered a record of their Immunisation history if they consider seeking vaccination by other clinics
Holiday Health
It is always a wise precaution to pack some essential items in case of illness on holiday. Do choose medicines according to your needs and the country you are intending to visit. lf you take prescription medicines regularly remember to pack them too.
You may want to consider packing the following:
- Paracetamol Travel Sickness tablets Plasters and a small crepe bandage
- Rehydration solutions such as Dioralyte, Anti diarrhoeal for example lmodium
- High factor sunscreen
- Anti-material tablets, Water purification tablets, Insect repellent
- Condoms and other contraceptives
- Sunhats for yourself and children

We offer most of the travel health service at no additional charge to the patient. However the NHS does not cover the cost of all the treatments you may need. Therefore, for some services you may need to pay.
