How To Register as a Patient

If you live in our practice area and wish to register as a patient at our surgery then we need certain information from you and each member of your family so as to be able to trace your previous medical records.

To register, call into the surgery bringing with you:

  • Your NHS medical cards from your previous NHS doctor.
  • 2 forms of ID, with one being in the form of photographic ID and the second being a proof of address.

I do not have my Medical Card?

If you do not have your medical card, you will need to complete form GMS1, you can do this when you visit, or download a copy from the forms section on this page.

This may arise if you cannot find your cards or if you do not have any e.g. if you have recently arrived in the country. This is less preferable since it may take longer to trace your medical records and you will have to fill in more paper work, so please find your medical cards if you can.

Registration Forms and Medical Questionnaire

When you register you will also be asked to fill out a Practice Registration form and a short questionnaire. This is because it can take a considerable time for us to receive your medical records. Download and print off the questionnaire (see forms section on this page), fill it out and bring it with you to the surgery. Alternatively you may wish to collect this from the surgery.

Health Check

An appointment for a health check performed by the nurse is offered to all new patients; an appointment can be made at reception.

Temporary Residents

The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.

To register you will need to bring your passport and complete our Temporary Resident Registration form which is available from reception, or can be downloaded from the 'Registration Forms' section on this page.

Practice Catchment Area

The practice catchment area is indicated on the sketch diagram below. For further details regarding the areas covered by our practice, please ask at reception. If you move outside the practice designated area you will be asked to move to a different practice.
