Introduction:
Holmcroft Surgery opened its present site in December 1994. For the previous 50 years it had occupied a site at
Lloyd Street off Gaol Road.
The new surgery provides modern premises allowing us to provide a wide range of services for the 10,000
patients who are registered with us.

Change of Patient Details:
If you change name, address or telephone number etc, please complete our
online form, or
let our receptionists know by telephoning or writing to us.
If you move outside of the practice area it will be necessary for you to find a doctor in
your new area.
South Staffordshire Primary Care Trust will help
you if you experience any difficulty. The instructions regarding moving to another area are
printed on your medical card.

Registering with the Practice:
If you live in the practice designated area you can join the list.
You will be asked to complete a Practice Registration form and a short questionnaire. An appointment for a health check
performed by the nurse is offered to all new patients; an appointment can be made at reception.
If you move outside the practice designated area you will be asked to move to a different practice.
Please keep us up to date if you change your name, address or phone number
(see above) as we may
need to contact you in an emergency.

Temporary Residents:
The doctors are happy to see patients who are temporary residents in our practice area
should they need medical help.

Suggestions & Comments:
We do try to ensure that we provide the best possible service at all times for our patients. If you would like to make a suggestion about any aspect of the surgery, please complete our online form or contact the Practice Manager or Patient Services Supervisor.

If you have a complaint about the surgery or the services we offer, please let us know.
You can contact the Practice Manager or Patient Services Supervisor or if you prefer, leave a note or letter at the reception.

Confidentiality:
If you wish to speak to the receptionist in confidence, there is a quiet area available away from the main reception. Please
ask.
Your personal details will be kept confidential by doctors and our staff members at all times. The practice is registered with
the Data Protection Agency and your computer records will be dealt with in accordance to their guidelines.

Information relevant to our surgery under the freedom of information act can be accessed by

you may print off a copy for reference if you wish. If you would like us to supply you with a paper copy of the
information please contact the Practice Manager.

Training of Junior Doctors:
Holmcroft Surgery has been a 'Training Practice' for junior doctors for some years now. These doctors are usually
training to be GPs and will spend anywhere from 4 to 12 months at the surgery. They have usually worked for 2-3
years in a variety of hospital specialities before they join us.
As part of their training they are asked to video some of their consultations with patients for discussion with their
Trainers (other doctors within the practice) and for their final examination. If your consultation is going to be videoed
you will be warned beforehand. If you would prefer for the consultation not to be videoed please feel free to say so. No
one will mind.

Facilities for Disabled Patients:
All facilities are ground floor and are designed for ease of access.
There is a fully equipped toilet for the disabled located at the entrance foyer. If you need assistance to enter the
building please ring the bell.
There is an induction hearing loop at reception for patients with hearing aids. Please ask reception if you need it.

Car Parking:
There is a large car park at the front of the surgery. There are reserved spaces for orange badge holders i.e. disabled
parking.
Please note that all vehicles are parked on the surgery car park at the owners risk.
